Cognitive Assessment

AXIA's Cognitive Assessment provides a detailed understanding of an individual's cognitive profile, including areas of strength and areas where additional support may be helpful.

Cognitive assessments may be useful alongside another AXIA assessment or where greater understanding of learning, processing or cognitive abilities is required.

• This service is available for adults and children.
• Private self-referral is available and no GP referral is required.
• Appointments can be provided online or in person, where appropriate.
• Pre-assessment questionnaires are provided and AXIA can assist with completing them.
• Normally one main appointment is required, although a screening call may sometimes be arranged beforehand.

Our team

AXIA is a family-run service established in 1998. Our multidisciplinary team includes Clinical Psychologists, Speech and Language Therapists, Occupational Therapists and a Mental Health Nurse.

Service delivery

This is a non-physical service and no shipping is required. Your appointment date and delivery format will be agreed directly with AXIA. Following the assessment, AXIA provides a detailed assessment outcome and report. Where appropriate, the outcome can be shared with relevant healthcare professionals and recorded within NHS records.
